A diode is reverse biased when its cathode (bar side) is at a higher potential than its anode.
(1) Anode connected to $+2\ \text{V}$ through the resistor, cathode at $0\ \text{V}$: forward biased.
(2) Anode at $0\ \text{V}$ (ground), cathode at $-5\ \text{V}$ through the resistor: forward biased.
(3) Anode at $+2\ \text{V}$, cathode at $-10\ \text{V}$ through $R$: forward biased.
(4) Anode at $+2\ \text{V}$, cathode connected to $+4\ \text{V}$: the cathode is at the higher potential, so the diode is reverse biased.
